Personal Information Change Form – For students changing their Legal or Chosen name, making corrections to their SSN, DOB, or address.  *Some changes can be made in the student Self-Service portal and some changes will require documentation. Please be sure to submit the required documents when submitting the form.

Audit PDF – For students requesting enrollment to audit a course. *Requires Instructor and Dean signatures.

Late Drop Petition PDF – For faculty submitting requests for a Late Drop.  *Must be submitted by the Instructor or Department Chair.

Loss of Priority Registration PDF- File a petition to appeal loss of priority status.

Pass/No Pass PDF – For students changing grading option to Pass/No Pass.  *Students are encouraged to speak to an academic counselor before requesting this change.

Refund Request PDF – For students requesting payment for refunds. *Submit to Business Services in SAS 310.

If a student has attained the age of 18 years or is attending an institution of postsecondary education (including K-12 students simultaneously enrolled in Butte College courses), the rights to access records shall therefore only be required of, and accorded to, the student.

The following “Directory Information” may be released without student consent: student’s name, participation in officially recognized activities and sports, weight and height of members of athletic teams, and degrees and awards received. The college also reserves the right to include information about expulsion on student transcripts. Students who do not want directory information released must inform the Office of Admissions and Records in writing on the appropriate form at the beginning of each term the information is to be withheld.  Please email admissions@butte.edu to request the form.
