Course Description: This course provides students with directed clinical experience in hospitals, community health agencies and agencies that provide services to the acutely ill adult, to the elderly or those having disabilities/chronic conditions. Through the use of the nursing process, the student will provide care, teaching, support, and rehabilitation to individuals who have acute or chronic illness, disabilities, or are elderly in the hospital or in the community. Emphasis will be upon the role of the registered nurse to promote illness prevention, community health, and positive aging and client independence. Pass/No Pass only.

Upon successful completion of this course, the student should be able to:

1. Formulate nursing diagnoses based on information gathered from a variety of sources.
2. Assess and evaluate in the clinical situation the combinations of signs, symptoms, diagnostic data, and/or disturbed communication patterns that signal possible existence of multiple, unstable and/or critical health care problems or psychopathology.
3. Formulate, plan and prioritize individualized care for clients based on nursing diagnoses.
4. Initiate and maintain care plans utilizing advanced nursing techniques to carry out nursing measures and delegated medical techniques.
5. Evaluate effectiveness of nursing interventions and short-term goals to apply appropriate modifications, based on assessment data, predetermined outcome criteria and ongoing client responses to the care delivered by the nurse.
6. Maintain accurate, effective, and therapeutic written, verbal, and nonverbal communications with clients, families, and members of the healthcare team, seeking guidance when appropriate.
7. Manage individualized care of clients utilizing theoretical principles of supervision and management.
8. Participate in collaboration and referral to facilitate client's return to the community.
9. Utilize the nursing process to implement and evaluate formal and informal teaching for clients, families, and communities in order to provide optimal nursing care to clients.
10. Provide care within legal and ethical parameters consistent with professional standards of registered nursing practice consulting with other health care professionals and advocating for clients when appropriate.

Reading Assignments

1. Given a patient with a complex chronic illness condition, research the condition in the medical surgical text and contrast and compare the actual patient's condition with the textbook description.
2. Read the chapter in the text book on neurologic trauma. Develop a plan of care to prevent and manage long term complications.

Writing Assignments

1. Write a two page pager analyzing the role of the team leader based on class discussions, reading assignments, and interview of a nurse team leader.
2. Using class handouts and readings from the text write a two page paper analyzing the safety of the home environment of your assigned home care client.
