DSPS Forms and Information

  • Intake Packet for DSPS Services  This packet contains all the forms you need to fill out to apply for services. Please fill out all fillable forms and print this entire packet to bring with you to your Intake Appointment. The forms are fillable online and there are several fields that you only need to enter into the application page and that information will also be entered onto the other forms making it easier to complete the forms quickly and efficiently. All of the items and documents you need to bring with you are listed below under the heading "What do I Need for my DSPS Appointment"
  • Disability Verification Form  Print this form to take to your doctor or other health professional that can verify and document your disability if you don't already have this required information. Verification of your disability is necessary in order for our office to determine your eligibility for our services.(A copy of this form is already included in the Intake Packet)
  • Consent to Share/Release Information  Fill out and submit this form to our office if you want to request that we send information to another college or other entity. (A copy of this form is already included in the Intake Packet)
  • K-12 to College Transition Information  This information will help explain the differences between how the K-12 system provides accommodation services and how accommodation services are provided at the community college.
  • Steps to Enrollment at Butte College. This check list will help ensure you have completed all the steps needed to get enrolled at Butte College and register for services with our program.
  • Courtesy Cart Reservation Form Eligible students need to make a reservation for the courtesy cart using this form at least 1 business day in advance.
  • Alternate Media Request Form Fill out this form if you have met with a Disabilities Specialist and are authorized Alternate Media services.
  • Special Class Repeats  Important information regarding steps necessary to repeat a DSPS Special Class.
  • Student Guide Brochure This brochure provides valuable information for students new to the DSPS program.

What Do I Need for my DSPS Appointment?

Student is responsible for providing the following documents:

  • A completed Intake Packet. (This means the entire packet  with a completed application, a completed consent form and a completed Exam Accommodation Request form (EAR).)
  • A printed copy of your current class schedule. (This verifies you are enrolled for the current semester at Butte College)
  • A printed copy of your most recent Unofficial Transcript. (This helps us determine where you are succeeding and where you may need additional assistance once it is determined you are eligible for DSPS services)
  • Printed copies of all documents that can verify that you are a student with a disability. (Some examples of acceptable documentation are: most recent IEP or 504 Plan from high school, medical report indicating what your disability is and how it affects your education, the Disability Verification form located in the Intake Packet above that has been completed by your treating physician or other medical professional. Some examples of documents that are not accepted for verification are SSI award letters, Dept. of Rehabilitation Award letters.) 
  • If you are under 18 years of age, a parent or legal guardian must be present at your appointment
