Documents are required for Federal Student Aid accountability. Documents are retained in the office for one year and then archived according to document-keeping requirements.

13-14 Board of Governors Fee Waiver Application
Waives an eligible student's enrollment fees (tuition). It is not necessary to file a separate BOGFW application if you have completed a FAFSA or the California Dream Application (for eligible AB 540 students).
13-14 Child Support Verification
To be completed by students required to verify either their or spouse's child support PAID, or their parent(s)' child support PAID.
13-14 Decline Pell Grant Form
Pell Grant is the foundational grant for eligible undergraduate students and is limited to 6 full-time years or 600% Lifetime Eligibility Used. This form is only for students who choose NOT to receive their scheduled Pell award for 2013-2014. Please give careful consideration to the choice you make.
13-14 Dependency Status Appeal (INITIAL REQUEST)
For applicants who would like to request review of their Dependent status for consideration as Independent for financial aid need analysis.
13-14 Dependency Override RENEWAL
For students who were previously granted a Dependency Override by Butte College. Students with initial requests must use the Dependency Status Appeal (see above) and provide documentation as requested on the form.
13-14 Eligible Non-Citizen Verification
Form required for students who are eligible non-citizens to document their non-citizen eligibility for Federal Student Aid programs.
13-14 Excess Units Petition
For financial aid recipients who have failed to meet the SAP time frame requirement. Students disqualified for exceeding time frame requirements may appeal by submitting an Excess Units Petition. Deadline for submitting petition: October 30 for Fall; March 30 for Spring.
13-14 FAFSA Dependency Status Form
Form required for students, who answered "yes" on the FAFSA to being orphaned, in foster care, a ward of the court, an emancipated minor, in legal guardianship, or homeless, to document their Independent status for financial aid purposes.

Form for students required to verify their identity and educational purpose. This form MUST BE PRESENTED, along with a valid government-issued photo ID, to a public NOTARY. The original form must then be mailed (no FAX or email) to Butte College Financial Aid Office, 3536 Butte Campus Drive, Oroville, CA 95965.
13-14 Income Tax Return Information Sheet for Special Filing Circumstances
Instructions for special filing circumstances: (1) filing extensions granted by the IRS; (2) amended IRS income tax returns; (3) tax filers who were victims of IRS identity theft; and (4) individuals who filed Non-IRS income tax returns.
13-14 Parent Income Tax Filer Verification
For students whose parent(s) are required to provide filed IRS income tax information for 2012.

Summer 2013 Pell Grant Application
For students enrolled in Summer Session 2013, who have a 2012-2013 FAFSA on file at Butte, and who have remaining 2012-2013 Pell eligibility. Students must register for Summer Session prior to applying for summer Pell Grant and be prepared to pay for classes before determination of aid eligibility.

Financial Aid Probation Contract
For students who have been notified that their SAP Appeal for Extenuating Circumstances has been approved, or for students approved for Probation due to term improvement. In order to receive financial aid while on Probation, students must return this completed form to the Financial Aid Office as soon as possible.

12-13 Satisfactory Academic Progress Extenuating Circumstances Appeal
May not submit before Feb. 11. Must be in classes and get Course Evaluation after 3rd week of semester. Course Evaluation is part of APPEAL packet. For financial aid recipients who have failed to meet SAP criteria. Extenuating circumstances may include serious illness or injury, death of an immediate family member, or other non-recurring catastrophic or compelling situation beyond the student's reasonable control. Appeals for SPRING 13 must be submitted AFTER the 3rd week of the semester (NOT before Feb. 11) and BY April 30, 2013.
