Registration

You will only need to register once per semester. There are 2 mandatory forms. (They can be downloaded, printed and then completed) Unfortunately, if these are not filled out, you will not be permitted to attend any training. Completing these forms allows us to provide FREE training to YOU!

A Brief Explanation of MANDATORY Forms:

1) Application for Admission to the College:

Download and print a copy of the Butte College Application for Admission. Please fill this form out as completely as possible, the entire 1st page along with all of Part I and V, on the 2nd page (A & B-I) of the application. There are no fees or units involved, this mandatory application covers liability. Unfortunately, if this is not filled out, you will not be permitted to attend the training.

2) 2011-2012 Participant Registration Form:

Download and print a copy of the 2011-2012 Participant Registration Form for the Foster and Kinship Care Education Program. Please fill it out completely. This mandatory form is very important; it is how we receive funding, which makes trainings such as these FREE to you!!

Please return all completed forms to the office staff when arriving for the training. Be sure to indicate the name and date of the training you wish to attend. You can also mail the forms to us at: 2491 Carmichael Dr. Ste. 300. Chico, CA. 95928. Or you may fax them to us at 530-897-6242.
